To schedule an evaluation, call (772) 222-5411. Below are the fees and payment terms described in Pinnacle’s HRT program materials.

What the initial visit covers

The $250 initial visit includes a physician evaluation and review of lab work. Treatment decisions depend on the provider’s medical judgment, clinical evaluation and laboratory findings. Enrollment does not guarantee a prescription, continued medication or a particular outcome.

What the monthly membership includes

The $179 men’s membership covers medical visits, ongoing HRT management and prescribed injectable testosterone or cream through Butterfield’s Pharmacy or Gardens Pharmacy. Medication is available for pickup or shipping.

Visits usually take place every four months, either in person or by telemedicine. Pinnacle’s program also accommodates a maintenance visit in addition to a scheduled appointment when needed.

Costs outside the membership

The membership excludes laboratory testing, imaging, additional procedures and medications obtained outside the program’s partner pharmacies. These expenses are separate from the listed membership fee.

Pinnacle’s HRT program is self-pay and does not accept insurance for its services. Patients may independently use insurance for lab work. The membership agreement is a medical service agreement, not an insurance plan.

How billing and payment work

The self-payment agreement specifies that the card on file is charged on the first business day of the month.

Payment is by credit or debit card; cash and checks are not accepted. A valid card must remain on file. The membership agreement states that fees are non-refundable and that failure to pay may result in suspension or termination of services.

Discounts listed in the program materials

Pinnacle lists a 10% discount for paying yearly in advance. It also lists a 15% discount for military veterans, firefighters, paramedics and police officers. With the 15% discount, the self-payment agreement lists the men’s rates as $213 for the initial visit and $152 per month. The two discounts cannot be combined.

Refills and laboratory requirements

Medication is prescribed as a three-month supply. Refills require account payments to be current, and Pinnacle requests at least two business days’ notice when a refill is needed.

The program instructions require lab work at least two weeks before the next visit. Missing required labs may delay or prevent treatment or refills.

Membership renewal and cancellation

The membership agreement states that membership renews automatically from month to month. Either party may terminate with 30 days’ written notice. The agreement also allows the practice to terminate membership for non-compliance, missed labs or inappropriate behavior.
